Hospital-to-Home

Our Hospital-to-Home (Door-through-Door) Recovery Support program helps seniors safely transition from the hospital back to their homes – ensuring the care and resources they need to heal are in place. By coordinating medical plans, home support, and essential services before, during, and after discharge, we reduce stress, improve recovery outcomes, and help prevent avoidable hospital readmissions.

Our dedicated Door-through-Door Coordinator works closely with seniors, families, hospital discharge teams, and community partners to identify risks, arrange services, and ensure a safe home environment. From securing medical equipment and in-home care to arranging transportation, meals, and follow-up appointments, we help remove obstacles so seniors can focus on what matters most – healing.
