There is no cost for our Referral and Support Advocate services, but donations are always welcomed and greatly appreciated. It is only through the generosity of our donors and partners that we are able to provide ongoing services and support to the seniors in our community

Standing Up for Older Adults

Our Referral and Support Advocacy program is dedicated to ensuring older adults can access the local resources and national entitlements they need to live safely, independently, and with dignity. We help remove barriers, educate individuals and families, and provide hands-on guidance to support healthier, more secure lives.

Whether someone is healthy and active or facing urgent and critical challenges such as food or housing insecurity, our goal is the same: to ensure every senior has the support they need to thrive. Our Referral and Support Advocates – experienced in social work, human services, and case management – take a personalized, compassionate approach to every situation.

When needed, our team conducts a comprehensive assessment that may include physical and mental wellbeing, food security, health conditions, mobility, home and environmental safety, financial and housing stability, and social or family support. We provide services directly whenever possible and collaborate with trusted community partners to offer a broad range of solutions. From simple referrals – such as an affordable local plumber – to complex assistance like applying for Medi-Cal, VA benefits, or Section 8, our advocates are here to help every step of the way

Advocacy Across a Variety of Services

  • Assisted Living – Recommendations, applications
  • Caregiving – Referrals for in-home, temporary and long-term, assisted living, skilled nursing care
  • Education – Technology classes and seminars particularly relevant to seniors – computer and phone help, phone scams, nutrition for aging, fall prevention, etc.
  • Elder abuse and neglect – Financial, physical and mental abuse, undue influence, caregiver neglect, self-neglect due to physical or mental issues
  • Financial assistance – Utilities assistance, medical bills, special needs, Fiduciary referrals
  • Health and wellness – Doctor visits, hospice info, home health options, vaccinations, medical insurance
  • Hospital recovery support – Coordination of care and resources post-hospitalization
  • Housing – Affordable housing, rentals, shared housing, Section 8 assistance, redeterminations, landlord mitigation, chronic homelessness, handyman and other home service referrals
  • Legal Issues – Understanding, referrals, and assistance with power of attorney, guardianship, fiduciary conservatorship, elder abuse, advanced medical directive, patient’s rights, restraining orders, civil and criminal issues, fair housing practices, undocumented issues, traffic court
  • Medi-Cal – Overview and application assistance, annual redeterminations
  • Medical equipment loans – Wheelchairs, walkers, commodes, home safety equipment, etc.
  • Mental health – Supportive referrals for mental and behavioral disorders, dementia, medication and alcohol dependencies
  • Nutritional needs – Help enrolling in immediate and ongoing programs, meal and food delivery, holiday meals
  • Safety issues – Home assessments, wellness checks, life alerts, fall prevention
  • Social Security – SSI overview and benefits, application assistance, Medicare support
  • Transportation – Local rides, out-of-town (health-related) transportation, wheelchair transport, rides to and from our Adult Day Care facility, grocery deliveries
  • Veterans’ programs – Benefits overview, application assistance, coordination between military and civilian agencies
